This form is used to report repairs needed for your room
(please submit only 1 request per form)

– This form is
used to report anything on campus that creates unsafe
conditions
Directions
for submitting a Work Order
- Click
on “Online Maintenance Work Order” or “Safety Work
Order” above (depending on your need)
- Under
“Department” use the scroll arrow and click on your Resident
Hall name to enter it
- Type
your first and last name in the “Name” space
- Type
your email address (This
is required, if you do not have one please use a friend’s)
- Type
your room number (Please remember to include letters, e.g. 104A)
- Type
your phone number
- Type
the description of work needed (Please submit only 1 request per
form)
- Scroll
the form down and click on "Submit Form"
You will receive an
email thanking you for submitting your request.
When the work order is completed, you will receive an email
notifying you of its completion.
